The NIMH’s Life Chart Methodology:

The National Institute of Mental Health created this chart. This is a practical way to help you keep track of the way you are feeling day-to-day, and to see what things make you feel better or worse. Print one out for yourself and give it a try. The NIMH refers to it as “Life Chart Methodology”, and it has been shown to be a valid and evidence-based tool (Denicoff, et al., 2000). Take it with you to any therapy sessions or show your medical doctor. This helps them stay informed. In those settings, it’s always 100% confidential!
